Your First Visit

Our goal for your first visit is to make it as stress free as possible. We provide printable forms you can read, fill out, and sign ahead of time in the comfort of your home, at your own convenience.

You can mail or email the completed and signed forms to our office along with photocopies of your ID and your insurance card prior to your appointment. This makes your check-in go smoothly and quickly during your first appointment. Don’t have time to complete these forms before your visit? Don’t worry, simply arrive 15 minutes early and relax in our reception area while completing the required documents.

Don’t Forget: Bring any previous dental records you would like us to review, your identification, and your insurance card (if one has been provided to you). You can relax in our comfortable reception area while we verify your information, make sure your forms are on file, and verify any insurance that will assist with payment.

Your new patient exam and consultation will take about 90 minutes. Dr. Butler and our team want to get to know you, not just your teeth. Our goal is to have you as a part of our patient family for life. We’ll also take a look at your teeth and gums with the following imaging processes:

  • Digital X-Rays. Dental radiographs can provide essential information about your oral health, and are an important part of your dental record. Our dental office uses an Orthoralix 8500 Digital Panoramic X-Ray System to help capture, store, and transmit digital dental radiographs – leading to greater accuracy in just one take, the ability to enhance images on a computer screen and not even the smallest signs of decay, and the tiniest possible amount of radiation exposure.
  • Intra-Oral Camera Imaging. Our intra-oral camera utilizes a small, sterilized probe-shaped camera which can be moved around the inside of your mouth, providing a computerized image of your teeth and gums enlarged and in living color on the computer screen. We can freeze and store the picture of the inside of your mouth for examination, then take an instant photograph for you and your records. This easy technique allows us and you to see worn-out, defective fillings; fractured or decaying teeth; bleeding gums; and plaque for yourself.

Once imaging has been performed, Dr. Butler will perform a complete head and neck cancer screening test, and complete a comprehensive chart of any existing dental work and restorations.

After the exam is complete, Dr. Butler will discuss his findings and any treatment options that he recommends. A complete treatment plan, with financing options, will be provided for your review and discussion.

Prior to the commencement of treatment, your anticipated fees and financial options will be reviewed with you. Acceptable forms of payment include:

  • Cash or Check
  • Visa – MasterCard – Discover – American Express
  • Care Credit

We provide reminders about your appointments in your preferred method, including email & text message reminders, postcards, and phone calls. Just let us know how you’d like to hear from us!
